    STATEMENT OF LIFE, HOPE, AND PALMETTO FELLOWS ELIGIBILITY

    I certify that I am not in default on a Federal Title IV or State of South Carolina educational loan, nor owe a refund on a Federal Title IV or State of South Carolina student financial aid program; that I have never been adjudicated delinquent; that I have not been convicted or pled guilty or nolo contender to any felonies and any alcohol or drug-related offense under the laws of this or any other state or under federal laws of the United States in order to be eligible for LIFE, Hope, or Palmetto Fellows scholarship; that I have not been convicted of any felonies and have not been convicted of an alcohol or drug-related misdemeanor offense during the preceding calendar year defined as 12 months from the date of the start of the school for period of this award; that I am a South Carolina resident; that I will be a full-time degree-seeking undergraduate student; (for incoming freshman) that I have a cumulative "B" average in high school under the Uniform Grading Scale. I have submitted previous and current official transcript(s) for all institutions attended; I understand that all previous and current grades received at institutions will be used in the calculation of my LIFE GPA. I hereby give permission for a background check to be conducted to verify the above. I understand additional information may be requested after the background check has been conducted. Should I use state aid in a summer semester of enrollment, I will also provide authorization to have my summer scholarship information made available to institutions for determination of continued eligibility.
    Any false information provided by the student or any attempt to expend any scholarship funds for any unlawful purpose or any purpose other than in payment or reimbursement for the cost of attendance at the institution authorized to award the scholarship and/or grant will be cause for immediate cancellation. Any student who has obtained a scholarship through means of a willfully false statement or failure to reveal any material fact, condition, or circumstances affecting eligibility will be subject to applicable civil or criminal penalties, including the retroactive loss of the scholarship and/or grant.

    SOUTH CAROLINA TUITION GRANT AFFIDAVIT OF ELIGIBILITY

    I certify that I have never been adjudicated delinquent or been convicted or pled guilty or nolo contendre to any felonies and have not been convicted or pled guilty or nolo contendre to any second or subsequent alcohol or drug related misdemeanor offenses under the laws of this or any other state or under the laws of the United States since one year prior to the first day of classes for the Fall 2019 Semester. If my status changes after signing this affidavit and before the first day of classes for the Fall 2019 Semester, I understand and agree that I must and will immediately report my adjudication, conviction, or plea to my college financial aid office and that I will lose eligibility for a 2019-2020 SC Tuition Grant.

    ANTI-DRUG ABUSE ACT CERTIFICATION

    I certify that I will not engage in the unlawful manufacture, dispensation, possession, or use of a controlled substance during the period covered by my aid.
